Summary:

We are searching for a Certified Diabetes Educator – someone who works well in a fast-paced hospital setting. You will be responsible to provide assessment, case management, education and ongoing management/monitoring to patients and families that promotes diabetes self-management in the inpatient and ambulatory settings. You will be responsible for development and review of policies and procedures that apply to diabetes care. Lastly, you will make routine decisions include resolving patient care issues at the unit level, implementing clinical protocols and providing complex diabetes management.

Responsibilities :

  • Teaches patient/family skills for diabetes self-management
  • Assesses patient family knowledge, individualized the education plan based on learner’s social, economic, cultural and age appropriate need
  • Evaluates patient needs at appropriate intervals and suggests appropriate management strategies
  • Documents all patient care related activities within the medical record according to established guidelines and formats
  • Develops and provides education materials and resources for staff and community
  • Evaluates current practice standards and evidence and develops and maintains resources. Maintains practice at appropriate levels by attending continuing education activities and researching evidence based practice
  • Implements proper protocols for diabetes patient management
  • Monitors and manages hypoglycemia and sick days per protocol, makes recommendations for medication dose adjustments per protocol, triages urgent calls per established guidelines
  • Provides case management related services including preauthorization paperwork


Qualifications:

  • Associate’s degree in Nursing or a Bachelor’s degree
  • One of the following:

    • Registered Nurse (RN) license issued by the Texas Board of Nursing OR Registered Dietician (RD) issued by the Commission on Dietetic Registration
    • Certified Diabetes Educator (CDE) issued by the National Certification Board for Diabetes Educators
  • Three years of professional clinical experience
  • A Registered Dietitian will also preferably be Licensed Dietitian (LD) issued by the Texas State Board of Examiners of Dietitians
  • Knowledge of diabetes management across the continuum of care
  • Ability to identify barriers to learning, learning preferences, and preferred/appropriate learning principles and age-appropriate instructional techniques
