Summary:

We are searching for a Care Management Assistant – Women's Health- someone who works well in a fast-paced healthcare setting. In this position, you’ll provide non-clinical administrative support to the Care Manager process including but not restricted to the following: timely discharges and utilization review, member follow-up appointments, member outreach, and sensitive customer telephonic interaction.

Responsibilities :

Duties

  • Provides department-related and other appropriate communications to internal and external customers 
  •  Provides assistance for utilization management processes to maintain compliance with Federal, State and Managed Care Guidelines 
  •  Provides assistance for discharge planning processes to maintain compliance with Federal, State and Managed Care 
  • Assists case management team with coordination of care across the continuum of care 
  • Guides and maintains data integrity


Qualifications:

Required

  • High school diploma 
  • 2 years of experience in hospital case management, home health, QI or a related field

 

 

Preferred

  • Some college course work 
  • Experience with Women's Health
  • Bilingual in Spanish and English
