About Texas Children's Hospital

Scheduled to open in 2017, Texas Children’s Hospital The Woodlands is a 548,000-square-foot, state-of-the-art free-standing pediatric hospital designed, equipped and staffed exclusively to treat children and adolescents in The Woodlands and counties throughout Greater North Houston such as Montgomery, Walker, Grimes, Liberty, Harris, Polk, San Jacinto and Hardin counties. Our new facility will include 25 emergency center rooms, 32 acute rooms, 28 critical care rooms, 12 radiology rooms and 4 operating rooms. We will also offer inpatient and outpatient pediatric care in more than 25 specialties including: cancer, cardiology, neurology, orthopedics, diabetes and endocrinology, urology, sports medicine, gastroenterology and nutrition, pediatric surgery, plastic surgery, otolaryngology, allergy and immunology, dermatology, adolescent medicine and physical rehabilitation. Summary:

We are searching for a Social Worker - someone who works well in a fast-paced hospital setting. In this position, you’ll assist patients and their families in making psychosocial adaptations specific to their diagnosis, while facilitating the optimal utilization of health care services and community resources in accordance to the age and developmental level of the patient.

Responsibilities :

  • Engages patient/family in problem solving as evidenced by documentation in the medical record and/or team observation/feedback
  • Makes and/or assists family to make resource referrals as evidenced by documentation in the medical record and/or team observation/feedback
  • Provides appropriate therapeutic support (individual/family/group/grief counseling, crisis intervention), either by direct service or by referral, as evidenced by documentation in the medical record and/or team observation/feedback
  • Conducts a psychosocial assessment, within one working day of receiving the inpatient consult or three working days of receiving the outpatient consult, as evidenced by documentation in the medical record and/or team observation/feedback
  • Communicates with the patient, family and team as evidenced by documentation in the medical record and/or team observation/feedback
  • Develops an action plan with the patient, family and team as evidenced by documentation in the medical record and/or team observation/feedback
  • Adheres to Departmental policies and procedures as evidenced by participation in department programs and observation by director and department staff
  • Identifies problems/needs in the department and participates in the problem solving process as evidenced by participation in department staff meetings, participation on a department committee and observation by director and department staff
  • Utilizes quality improvement process to identify improvement opportunities and establish/assess/evaluate programs and services and their outcomes as evidenced by observation by director and department staff
  • Participates in department performance improvement program as evidenced by PI coordinator, director and department staff observation


Qualifications:

  • Master’s degree in Social Work from an accredited School of Social Work
  • Licensure as a Social Worker in the State of Texas
  • Ability to conduct a thorough psychosocial assessment and develop, implement and evaluate a plan of care
  • Ability to conceptualize the psychosocial needs presented by the assigned patient population, to intervene with patient/family behavior, to coordinate community resources and to handle crisis and complex psychosocial situations
  • Ability to handle grief
  • Ability to demonstrate initiative, good judgment and problem solving skills.
  • Ability to demonstrate excellent professional listening and communication skills.
