We are searching for an Access Care Manager — someone who works well in a fast-paced setting. In this position, you'll function as a member of the interdisciplinary healthcare team in the provision of patient care with the underlying objectives of enhancing the quality of clinical and financial outcomes and patient satisfaction while managing the plan of care.

Responsibilities :

  • Screens patients for level of care and ensures clinical information in the medical record is clear and accurately reflects level of care requested
  • Collaborates with the medical, nursing, and ancillary staff to coordinate delivery of high quality, individualized, and cost-effective patient care
  • Ensures quality, cost-effective use of resources and facilitates access to care
  • Serves as a liaison between members of the interdisciplinary care team
  • Coordinates appropriate and timely placement of internal and external transfers for all patients within the acute care setting
  • Oversees patient access to optimally assure clinical and financial outcomes
  • Demonstrates sound, effective, interpersonal, communication, and professionalism skills



·         Associate or bachelor’s degree in science in nursing or technical diploma in nursing

·         3 years of nursing experience as a registered nurse in an acute care setting

·         Pediatric and case/care management experience preferred

·         Registered Nurses License issued by State of Texas Nursing Board

·         Basic Life Support certified by American Heart Association

·         Certified Case Manager preferred

·         Knowledge of specific nursing processes, anatomy, and physiology

·         Excellent verbal and written communication skills

·         Analytical decision making and judgment skills

·         Demonstrated ability to function as a clinical care team leader

·         Knowledgeable of all clinical resources available to patients both inpatient and outpatient

·         Data entry and word processing skills
