We are searching for a Care Manager - someone who works well in a fast-paced hospital setting. In this position, you’ll function as a member of the interdisciplinary healthcare team in the provision of patient care with the underlying objectives of enhancing the quality of clinical and financial outcomes and patient satisfaction while managing the plan of care. Within the purview of financial management, you will proactively communicate with members of the healthcare team to ensure appropriate medical necessity for patients in beds, as well as communicate with payors and the patient/family/health care team regarding payor information as necessary.

Responsibilities :

  • Complete a comprehensive problem identification analysis to identify opportunities for intervention that are appropriate and realistic for the patient/family’s psycho-social, cultural, spiritual, and physical plan of care 
  • Coordinate, facilitate, and monitor the plan of care for patients throughout the episode and continuum of care to achieve the optimal clinical and financial outcomes 
  • Oversee discharge planning and facilitate safe transitions to community settings 
  • Serve as a liaison between members of the interdisciplinary care team, community providers, payor sources, and the patient/family to ensure safe and effective plans and smooth transitions between internal and external levels of care 
  • Effectively and efficiently oversee the plan of care to optimally assure clinical and financial outcomes


  • Bachelor’s or associate’s degree in nursing, or technical diploma in nursing with acceptance or current enrollment in BSN program and signed agreement
  • Registered Nurses License by State of Texas Nursing Board
  • Certified Basic Life Support by American Heart Association
  • Certified Case Manager
  • 3 years of experience in position requiring RN licensure in an acute care setting
  • Pediatric and case/care management experience preferred
  • Knowledge of specific nursing processes, anatomy, and physiology
  • Excellent verbal and written communication skills
  • Analytical decision making and judgment skills
  • Demonstrated ability to function as a clinical care team leader
  • Knowledgeable of all clinical resources available to patients both inpatient and outpatient
  • Data entry and word processing skills
