Summary:

We are searching for an Assistant Director of Care Coordination (Patient and Family Services) — someone who works well in a fast-paced setting. In this position, you will manage and oversee daily clinical, administrative, and human resource activities for all designated Care Management area(s), including providing 24-hour availability. You will participate, coordinate, and ensure the delivery of high-quality operational activities that are directly associated with care coordination and utilization management of medical services provided to patients and families. Lastly, you will ensure that developmentally appropriate patient and family-centered care is being provided at all levels in designated areas.

Responsibilities :

  • Create and facilitate collaboration and cooperation among all care coordination, utilization management functions, and other professional disciplines to achieve desired results
  • Utilize multiple sources of information such as risk stratification, tier level, predictive analytics and reports to identify patient resource consumption trends and action plan development
  • Demonstrate fiscal accountability for areas of responsibility, as evidenced by the appropriate management and justification of resources, which includes personnel, equipment, supplies, and capital
  • Partner with the director and multidisciplinary teams to guide target setting processes for achieving and improving the quality of care
  • Facilitate staff participation and participate in the Quality Outcomes Program and/or unit-based quality initiatives to develop and monitor exemplary clinical outcomes
  • Commit to the on-going development of self, unit leadership team and staff to achieve professional goals

 

 

 

 

 



Qualifications:

 

  • Bachelor’s degree in nursing 
  • Master’s degree in nursing, business or healthcare field
  • Licensed Registered Nurse from the Texas Board of Nurses
  • Basic Life Support Certificate issued by the American Heart Association
  • Minimum 3 years’ experience in management or leadership
  • Minimum 3 years’ experience in care coordination experience
