We are searching for Social Worker — someone who works well in a fast-paced setting. In this position, you will assist patients and their families in making psychosocial adaptations specific to their diagnosis, while facilitating eligibility determination for social programs and funding resources.

Responsibilities :

  • Develops and initiates the psychosocial plan of care
  • Coordinates, implements, and follows-up on the psychosocial plan of care
  • Engages and empowers patient/family in problem solving and decision making with regard to their health care decisions as evidenced by documentation in the medical record and/or team observation/feedback
  • Contributes to the efficient functioning of the department
  • Participates in program development for purposes of improved quality of care and service delivery
  • Pursues professional development
  • Provides evidence based and client centered therapy to help patients and their families cope with their medical, behavioral or mental health conditions


  • Master’s degree in social work
  • Licensed Master Social Worker, Clinical Social Worker, and Licensed Master Social Worker- Advanced Practice by the Texas Health and Human Services Commission
  • Child Protective Services (CPS) experience Preferred
  • Ability to conduct a thorough psychosocial assessment and develop, implement, and evaluate a plan of care
  • Ability to conceptualize psychosocial needs
  • Ability to intervene with patient/family behavior and coordinate community resources
  • Abiliity to handle crisis and complex psychosocial situations
  • Ability to handle grief and bereavement situations
  • Demonstrate initative, critical thinking, good judgment, and problem solving skills
  • Demonstrate excellent professional listening and communication skills
