We are searching for a Social Worker— someone who works well in a fast-paced setting. This role will assist patients with chronic, complex and/or life-threatening conditions and their families in making psychosocial adaptations specific to their diagnosis while utilizing health care services and community resources. This position will provide clinical social work and therapeutic services to palliative patients and families using a collaborative approach. The social worker position will assess, plan, implement, deliver, monitor and evaluate the options and services required to meet an individual's health needs.

Responsibilities :

  • Develops and Initiates the Psychosocial Plan of Care
  • Develops, implements and/or updates plan of care
  • Facilitates patient/family use of community resources
  • Participates in program development for purposes of improved quality of care and service delivery
  • Participates and pursues professional and program development


  • Master’s degree in Social Work required
  • LSW – Social Worker License from the state of Texas required
  • 3 years of experience  working with patients with life-threatening conditions, chronic disease, hospice and/or end-of-life care and related situations is preferred
  • Pediatric experience preferred
  • Experience working with family systems preferred
