Summary:

We are searching for a Field Relations Coordinator – someone who works well in a fast-paced setting. In this position, you will be responsible for the deployment of team-based population management including developing and maintaining positive relationships with physicians, members, their families, and providers. Additionally, you will interact with physicians, members, caregivers, and the care coordination team on a daily basis. You will also be responsible for assessments, coordination activities, referral and linkage to community resources, and monitoring and tracking outcomes.

Responsibilities :

  • Develops relationship with participating providers
  • Collaborates with care coordination team and provider to develop, implement, and monitor member care plan
  • Assists family with obtaining appropriate medical and community resources
  • Develops support systems to meet the members’ needs by identifying and coordinating a variety of available services based up member needs and preference
  • Track and monitor medical, behavioral, and community resources
  • Travels to member homes for periodic evaluation and assessment of unmet functional needs, educational needs, and goal attainment seeking member/family input in care plan
  • Ensures provider satisfaction
  • Delivers exceptional customer service to contracted providers and members
  • Ensures thorough communication and education to members/ providers an understanding of delivery system capabilities to improve access and address member needs
  • Actively contributes relevant knowledge during team meetings and contributes to program direction through special projects


Qualifications:

  • High school diploma or GED required
  • Driver’s license from any state and auto insurance required
  • 5 years’ experience in healthcare, local and state programs for children with disabilities or managed healthcare setting required
  • Bilingual preferred
  • Community Health Worker preferred
