Texas Children’s Hospital has begun its expansion from Houston to Austin! We are excited about the opportunity to extend Texas Children’s mission to a large and growing population of children and women. Our Urgent Care opened in March 2018, several pediatric practices will open this spring and summer, and our first Specialty Care Center will open this October. The Specialty Care Center is located at 8611 North Mopac Expressway, approximately 8 miles north of downtown Austin near Highway 183 and will include these subspecialties and more: • Cardiology • Pulmonary Medicine • Neurology • Diabetes/Endocrinology • Gastroenterology • Dermatology • Allergy & Immunology • Urology • Ophthalmology • Plastic Surgery. Our robust, outpatient-focused approach includes a plan through 2021 for 18 pediatric practices, 4 urgent cares, 3 specialty care centers, and 2 maternal fetal medicine centers, all on 1 integrated medical record. Summary:

We are looking for a  Medical Social Worker -- someone who works well in a fast-paced setting. In this position, you will assist patients and their families in making psychosocial adaptations specific to their diagnosis, while facilitating eligibility determination for social programs and funding resources. You will apply optimal utilization of healthcare services and community resources in accordance with the principles of human growth and development and provide clinical social work and therapeutic services to our patients and families by utilizing a collaborative approach. You will also assess, plan, implement, deliver, monitor and evaluate the options and services required to address a patient's individualized plan of care in collaboration with the interdisciplinary team to achieve discharge goals.

Responsibilities :

• Conducts a psychosocial assessment, evaluating patient and family psychosocial risk factors
• Provides case management as needed for psychosocial continuity of care as evidenced by documentation in the medical record and/or team observation
• Engages and empowers patient and families in problem solving and decision making regarding their healthcare decisions as evidenced by documentation in the medical record and/or team observation
• Contributes to the efficient functioning of the department
• Participates in program development for purposes of improved quality and service delivery
• Actively participates in hospital performance improvement initiatives and regulatory agency preparedness to ensure improved and sustained performance as evidenced by the leadership team observation
• Educates self by reading professional literature and attending staff development and continuing education programs as evidenced by attendance and CEU records
• Provides evidence-based, and client-centered therapy to help patients and their families cope with their medical, behavioral, or mental health conditions HOU123

Qualifications:

• Master’s degree in social work required
• LMSW, LCSW, or LMSW-AP certification from the Texas Health and Human Services Commission required
• Previous employment in healthcare, healthcare specialization, or a field work practicum is preferred
