We are searching for a Patient Care Manager- Outpatient- TCHPC - GREENSPOINT PEDIATRICS; someone who works well in a fast-paced setting. In this position, you will participate in the delivery of high quality, family-centered and developmentally appropriate patient care at the clinic level. The Patient Care Manager provides leadership to direct clinic patient flow, staffing and clinical quality in collaboration with physician and administrative outpatient leaders.

Responsibilities :

  • Coordinates and collaborates outpatient clinical operations and patient care needs with appropriate members of the healthcare team to achieve desired clinical outcomes and management of care across the continuum.
  • Monitors current and projected patient volumes and clinical processes via data collection and observation.
  • Effectively and succinctly compiles, analyzes, and presents data to diagnose process improvement opportunities and goals to the team and department.
  • Observes, identifies, and addresses human resources issues for assigned staff.
  • Coaches, counsels, and develops staff to ensure the delivery of high-quality patient care.
  • Maintains clinical competency by ensuring that the physiologic, psychosocial, and developmental needs of the patient are met within a family-centered care environment.
  • Ensures staff development and employee onboarding occurs within the guidelines set by the Department of Ambulatory Services.
  • Conducts annual learning needs assessment (in collaboration with the education coordinator) for direct reports and submits to Ambulatory leadership for annual professional training curriculum planning.


  • Experience in Word processing, Excel, and Power Point required.
  • Knowledge of quality/process improvement concepts.
  • Experience of outpatient clinic operations.
  • Bachelor’s Degree in Nursing required.
  • 2 years of nursing experience required.
  • Nurse registration/license and certification in basic life support required.
