We are searching for a Clinical Program Coordinator to work in our Rheumatology department-- someone who works well in a fast-paced setting. In this position, you will coordinate all activities related to the clinical program for several targeted patient populations both inpatient and outpatient. This includes but is not limited to coordination of all activities related to evaluation and consultation, coordination of physicians / or teams for surgical interventions, family communication and education, coordination of multidisciplinary team to assess and care for the patient, care of the inpatients, discharge planning, outpatient follow-up as well as pertinent data management for the program / clinic for research, outcomes and/or reporting purposes. This role will also involve working closely with our nursing team and providers for coordination and flow of the outpatient clinic visits.

Responsibilities :

  • Provides education to patients, families, communities and other hospital/medical staff
  • Coordinates all activity for identification, evaluation, and presentation of potential patients according to Departmental policy as observed by Clinic Chief or attending physician
  • Accurately maintains scheduling of outpatient appointments, including consultant evaluations, necessary testing, and visits in a timely and efficient manner
  • Provides patients and families consistent and complete follow-up information including signs and symptoms of difficulties, and follow-up appointments on an ongoing basis as observed by Clinic Chief or designee’s review
  • Acts as a patient advocate in maintaining communication and promoting continuity of care between clinical program and referring and community physicians
  • Maintains a database of potential patients
  • Promotes education about the program to physician groups as well as in the community as observed by clinic chief or designee


  • Graduation from an educational program applicable to the program specialty
  • One or more of the following certifications are required: LMSW, LCSW, LMSW-ACP, LMSW-AP or LBSW from the Texas State Board of Social Worker Examiners, RD certification from the American Dietetic Association, LD license from the Texas Department of Licensing, PT or OT certification from the Texas Board of OT Examiners, CCC-SLP, TX-SLP, or TXAUDLIC from the State Board of Examiners for Speech-Language Pathology-Texas, LPC from the Texas State Board of Examiners of Professional Counselors
  • 3 years licensed clinical experience required
  • Preferred experience as a Licensed Social Worker
