We are looking for a Manager Diabetes Care and Education-  an experienced Certified Diabetes Care and Education Specialist to lead diabetes care and education specialists, endocrine dietitians and other clinical support staff that deliver diabetes self-care management and education services for our pediatric and maternal patients.  .

Qualifications:

  • Bachelor's Degree Nursing and  Master's Degree is required if using an RN license to qualify for the role. 
  • If using an LD license to qualify for the role, only a Master's Degree is required. 
  • RN - Lic-Registered Nurses Texas Board of Nursing or Nurse Licensure Compact Or LD - Licensed Dietitian Texas Department of Licensing and Regulation required
  • CDCES - Certified Diabetes Care and Education Specialist Certification from Board of Diabetes Care and Education Or BC-ADM - Cert-BC Adv Diabetes Management Certification Board of Diabetes Care and Education Experience Required
  • 4 years Diabetes self-management education experience required

Responsibilities:

  • Ssuccessful recognition by the American Diabetes Association for Diabetes Self-Management Education across the system through coordinated efforts with leaders and the program quality coordinator
  • Supporting a positive team experience, the leader will participate in weekly culture rounding, ensure adequate staffing, evaluate staff performance and provide effective employee development
  • Will establish annual goals to address gaps or improve outcomes related to the diabetes program
  • Collaborates with community partners to establish and maintain on-going support options.
  • Develop efforts for quality and practice improvement and ensure compliance to Joint Commission standards
